The Chronic idiopathic pancreatitis rich in xanthomatous histiocytes and post-traumatic abscess
A case report with atypical presentation

This article presents a case report of chronic idiopathic pancreatitis rich in xanthomatous histiocytes associated with post-traumatic splenic abscess, a rare and atypical combination. The patient, a 37-year-old male, presented with upper abdominal pain and nausea following an episode of food and alcohol consumption, with persistent pain lasting thirteen days. Imaging studies revealed pancreatic and splenic involvement, leading to a body-caudal pancreatectomy and splenectomy. Histopathological and immunohistochemical analysis confirmed chronic inflammation with xanthomatous histiocytes and splenic necrosis. Surgical intervention was essential for clinical management and patient stabilization; who showed a positive post-operative recovery and was discharged. This case highlights the importance of differential diagnosis in atypical presentations of pancreatitis and splenic abscesses and the relevance of surgical management to ensure patient quality of life.
